Responsible for assessment, member-centered care plan development, ongoing care coordination, quality assurance, and monitoring of services to program members, who may be older adults and/or individuals who have intellectual and/or physical disabilities.
- Achieve member outcomes and goals through adherence to standards of care and best practices
- Interact with members and their families with the highest level of respect and compassion, and participate in efforts to continuously improve the quality of member care and services
- Promote the member-centered culture
- Complete discipline-specific initial and ongoing assessments, conduct home visits and home assessments as needed, and consult with families and other supports for additional information as needed
- Develop and maintain care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports
- Educate members and families regarding aging, disability, or health issues, and provide information to members to assist in establishing and maintaining community links. Assist members with filing complaints, grievances, or appeals and obtaining advocacy services as needed.
- Maintain member contact standards and documentation standards, and stay informed of current trends and best practices
Manage an assigned caseload of program members in collaboration with an RN Care Manager. Complete comprehensive member assessments, member-centered care plans, and functional screens. Provide ongoing care coordination and quality assurance monitoring of services. Maintain required documentation in adherence to established standards. Typically handling 40+ members.

Qualifications - Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
- Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
Work Environment Primarily day shift hours. 8am to 4:30pm. Monday through Friday. Training period of 3-6 months in-office before transitioning to a hybrid schedule. Regular local travel requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Expected to be in the office generally 3 days per week. Visits are required in home and/or at LTC facilities must be willing to travel through out the county to conduct visits.
